When my patients come in for an exam, they are each required to update their health history forms and notify me of any changes in their health status. For the children that I examine, there is a simple question asking parents if there is anything about their child's mouth that concerns them.
I found this form recently when preparing to go in and examine a little 10yr old boy. Apparently he was struggling with hygiene and his mother wanted me to address the issue...

Reminds me of my last trip to the dentist. A lady was with her son on the other side of the wall. All I could hear was her yelling "If you'd FLOSS this wouldn't happen!!!" over and over again. I could tell that the dentist was just itchin' to get over there!
